New service may be initiated by one of the following options. You may also call our office @ 901-385-5585 for information.
Option 1 (Remote): Fill out this Form to request an application be sent to you to complete and sign electronically. You may also call our office to request an online application be sent to you.
Option 2 (Download & visit): Click on one of the following forms, fill out, print and bring to City Hall along with required documents.
Option 3 (Onsite): Come into City Hall to sign up for service and bring required documents.
Required Documents for initiating service:
TWO (2) FORMS OF IDENTIFICATION FOR EACH APPLICANT FROM THE LIST BELOW ARE REQUIRED IN ORDER TO ACTIVATE YOUR ACCOUNT AND COMMENCE WATER SERVICE. THE FOLLOWING FORMS OF IDENTIFICATION, ONE OF WHICH MUST BE A PICTURE FORM OF IDENTIFICATION, ARE ACCEPTABLE: OFFICIAL DRIVER'S LICENSE OR STATE ID CARD (ISSUED BY STATE AS ID IN PLACE OF DRIVER’S LICENSE) • FEDERAL IDENTIFICATION CARD (PASSPORT AND/OR VISA, ALIEN REGISTRATION CARD) • MILITARY DISCHARGE PAPERS • VOTER’S REGISTRATION CARD • CERTIFIED COPY OF THE BIRTH CERTIFICATE • IRS INDIVIDUAL TAXPAYER IDENTIFICATION NUMBER CARD • MUNICIPAL, STATE OR FEDERAL EMPLOYMENT ID • MEDICAID OR MEDICARE IDENTIFICATION CARDS • STATE-ISSUED HUNTING AND FISHING LICENSE • STATE-ISSUED GUN PERMIT OR LICENSE • SOCIAL SECURITY CARD • EBT (ELECTRONIC BENEFITS TRANSFER CARD) • CONSULAR ID CARDS.
